How it works

Fotona 4D is a comprehensive non-surgical facelift protocol developed by Fotona, a European laser manufacturer with over five decades of clinical laser engineering. The 4D protocol uses two of Fotona's signature laser wavelengths, the 2940nm Er:YAG and the 1064nm Nd:YAG, to deliver four sequential treatment steps in a single session. The result is a multi-layered rejuvenation that lifts, tightens, and refreshes the face without injectables, surgery, or significant downtime.

The first step, SmoothLiftin, delivers Er:YAG laser energy through the inside of the mouth, treating the deeper tissues of the cheeks and perioral area from the inside out. This intraoral approach is one of the most distinctive features of the Fotona 4D protocol because it volumises and tightens facial structures that other lasers cannot reach. The second step, FRAC3, uses the Nd:YAG wavelength in a fractional pattern to target imperfections in the deeper skin layers including pigment, vascularity, and signs of laxity.

The third step, PIANO, applies a long, gentle bulk-heating pulse with the Nd:YAG laser. This sustained warming stimulates collagen contraction at depth and triggers a longer-term collagen remodelling response. The fourth and final step, SupErficial, uses a light Er:YAG peel pass to polish the surface of the skin and improve overall tone and texture. Together, the four steps treat the face at four different depths in a single coordinated session.

The Fotona platform is CE marked and Health Canada approved for a wide range of dermatological and aesthetic indications. The 4D protocol specifically is well known for producing visible lifting and contouring effects that mimic a surgical facelift over a series of treatments. A standard Fotona 4D session takes 60 to 90 minutes, with mild redness as the only typical short-term effect. Most patients return to normal activities within hours.

What are the four steps in a Fotona 4D treatment?


The protocol moves through four sequential steps. Step 1 (SmoothLiftin) delivers laser energy through the inside of the mouth to tighten and volumise the cheeks from the inside out. Step 2 (FRAC3) targets imperfections in the deeper layers of the skin, including pigment, vascularity, and laxity. Step 3 (PIANO) uses a long, gentle bulk-heating pulse to stimulate collagen contraction and remodelling at depth. What does a Fotona 4D session feel like?


Each step has a different sensation. The intraoral step feels like a warm pressure inside the mouth. The deeper laser steps produce a sensation of warmth on the skin with occasional brief snaps. The bulk-heating step feels like a sustained warming, often described as relaxing. The surface peel step is the most superficial and feels like a light, brisk pulse. Numbing is generally not required.

How long is a Fotona 4D session?


A full Fotona 4D session typically takes 60 to 90 minutes, including all four sequential laser steps. This is a thorough treatment by design, not a quick procedure, because each step is doing distinct work at a different skin depth.

Is there downtime after Fotona 4D?


Downtime is minimal. Most patients experience mild redness and a flushed appearance for a few hours to one day. Makeup can typically be applied 24 hours after treatment. There is no peeling, scabbing, or social downtime in most cases. Your practitioner will provide a clear post-treatment care plan.

When will I see results?


Many patients notice an immediate tightening and brightening effect after a single session. The most significant lifting, contouring, and skin quality improvements develop over the following two to four months as collagen remodelling continues. Results build cumulatively across a series of treatments.

How many sessions will I need?


Most patients see meaningful improvement after a series of 3 to 4 sessions spaced four to six weeks apart, followed by maintenance treatments every six to twelve months.
