Under-eye concerns treatment in Metro Vancouver

Tired-looking eyes have several different causes, and each one answers to a different treatment. Naming the cause is where good under-eye care starts.

Understanding the condition

What under-eye concerns actually are

Under-eye concerns are several separate conditions that share one small patch of skin and need different treatments.

The skin under the eye is the thinnest on the body. Very little sits between the surface and the muscle, bone and blood vessels underneath, so small changes show quickly, and more than one change often shows at once. What people call dark circles, eye bags or tired eyes usually comes down to one or more of five causes, and the treatment that helps depends entirely on which ones you have.

Naming the cause

The five causes of under-eye concerns

Most people have a mix of these, not just one. Sorting out which ones are yours is the whole point of the consultation.

Pigment (periorbital hyperpigmentation)

Extra melanin in the skin itself, brown in tone. It stays dark when the skin is gently stretched, often runs in families, and is more common in medium and deeper skin tones. Filler does nothing for it, because the brown is in the skin, not a shadow under it.

Vascular, thin skin showing vessels

A blue, purple or pink tint from fine blood vessels seen through translucent skin. It is worse with fatigue, allergies and dehydration, and it lightens when the skin is pressed. The aim is to thicken the skin so what lies beneath shows less.

The tear trough, a structural hollow

A groove between the lower lid and cheek, deepened by age-related loss of fat and bone. It casts a shadow that reads as darkness even when the skin colour is perfectly even. This is the hollow that filler is designed for.

Eye bags, fat herniation

The fat pads that cushion the eye push forward into a bulge as the membrane holding them weakens, often with loose skin over it. No cream, laser or filler flattens herniated fat. When bags are the main problem, the honest answer is usually surgical.

Fluid puffiness

Temporary swelling from salt, alcohol, poor sleep or allergies. It comes and goes, is often worse in the morning, and answers to sleep, sleep position and allergy control rather than to any treatment we could sell you.

The BioPlus difference

Why under-eye darkness differs on deeper skin

Pigment-type dark circles are far more common, and more visible, in medium and deeper skin tones. Periorbital hyperpigmentation, the medical name for pigment dark circles, is one of the most frequent skin complaints among South Asian, Middle Eastern and East Asian patients and in Fitzpatrick types IV to VI, and it often runs in families. For many people it is constitutional, simply part of how their skin is made rather than a sign of tiredness, poor health or anything they did.

When the darkness is pigment, the tools that fix a hollow do nothing for it. Filler placed under pigmented skin leaves the brown exactly where it was, now sitting on a fuller cushion. That is the single most common under-eye disappointment we see, and getting the cause right the first time is what prevents it.

The cause decides the tool

Filler placed under pigmented skin leaves the brown exactly where it was. The most common under-eye disappointment is a pigment problem treated as a volume problem, so naming the cause first is the whole game.

Pigment on deeper skin, treated gently

The pigment cells in deeper skin tones react strongly to heat and injury, so an aggressive laser can leave more pigment than it removes. Sun protection and pigment-directed skin care come first, and energy devices are used cautiously and tested on a small area.

Options that carry no product pigment

Treatments that use your own blood or fine needles carry no pigment from a product and use as few punctures as possible, which is a genuine advantage on darker skin.

Some under-eye problems are not ours to treat, and we will tell you so

Filler cannot fix pigment, and it cannot flatten an eye bag made of herniated fat. When bags or loose lower-lid skin are the main problem, the honest answer is a referral to an oculoplastic or plastic surgeon, not a syringe that cannot flatten a bulge. Darkness driven by an allergy or a thyroid condition belongs with your family doctor first. Saying this plainly is part of the assessment, not an afterthought.

We would rather send you to the right person than treat you with something that cannot work.

How your visit works

From first look to the right plan

Under-eye treatment fails when it treats the wrong cause. So the consultation spends its time on the diagnosis first, before it talks about any treatment.

1
Consultation

A physician looks at the area in different lighting, gently stretches the skin to separate pigment from shadow, and presses it to see whether the darkness comes from vessels. A Wood's lamp reads pigment depth where it helps.

2
Your plan

We name the cause or causes, then match the plan to them: pigment-directed care, skin-quality work, a small and conservative amount of filler for a true hollow, or an honest referral where that is the right answer.

3
Treatment

Staged and conservative, sequenced so reactive skin is never provoked into more pigment. Tear-trough filler in particular is placed by an experienced medical injector, in small volumes.

4
Follow-up

Baseline photographs let us measure change instead of guessing it, and pigment work is reviewed over months, because it fades gradually rather than in a session.

How care is built

Your plan follows the cause

No two under-eye plans at BioPlus are the same, because the plan follows the cause the consultation finds.

01
The cause leads

Once the consultation has named what drives your darkness, the plan points in one or more clear directions rather than a default. This is the step other pages skip.

02
Skin quality first

For pigment and thin-skin darkness, sun protection, pigment-directed skin care and gentle collagen-building work are the part that holds any result. This layer comes first and it is the one that lasts.

03
Filler only where it fits

A small, conservative amount of hyaluronic acid filler lifts a true tear-trough hollow so it stops casting a shadow. It is used only when a hollow is the cause, and never for pigment or eye bags.

Treatments we may use

The tools we reach for under the eye

Which ones appear in your plan depends entirely on the cause the consultation finds, and some under-eye concerns are best served by none of them.

Dermal fillers

Hyaluronic acid filler

Injectable gels of hyaluronic acid, a water-binding sugar the body already makes, and the best-evidenced treatment for a true tear-trough hollow. Placed in a small amount, deep on the bone by an experienced medical injector, a soft filler lifts the groove so it stops casting the shadow that reads as a dark circle. A 2023 review found high satisfaction and low rates of serious complications.

Why this oneFor a genuine hollow in skin of good quality, never for pigment circles, eye bags or marked laxity.

Microneedling

Fine needles, no heat or light

A pen of fine needles makes thousands of controlled micro-channels in the skin, with no heat and no light. Under the eye its job is to thicken and firm thin, crepey skin so underlying vessels show less, and to help pigment-directed products absorb. Because it produces no heat and targets no pigment, it is one of the gentlest collagen options across every skin tone.

Why this oneA first choice for thin, crepey, mildly pigmented skin, including patients turned away from lasers for their skin tone.

PRP & PRF rejuvenation

Your own blood, growth factors

A small blood sample is spun to concentrate the platelets, which carry growth factors that signal repair, then the concentrate is injected back into the under-eye skin. Small trials report improvement in dark-circle pigment, texture and fine crinkling over a course, and because the material is your own blood there is no product pigment risk, a real advantage on darker skin.

Why this oneA natural option for early changes and thin skin with visible vessels, or when filler does not suit.

Under-Eye Refinement

Physician-designed under-eye program

A physician-designed under-eye program whose value comes from the order it works in: find the cause first, then match the treatment to it. A structural hollow may be treated with a small amount of tear-trough filler, thin skin with microneedling or platelet therapy, and pigment with pigment-directed care and sun protection rather than filler. Eye bags from herniated fat are referred for a surgical opinion, honestly.

Why this oneFor anyone bothered by shadows, hollows, fine crinkling or tired-looking eyes who wants a plan built around the cause.

What to expect

Sessions, downtime and results

Timelines

A tear-trough hollow treated with filler looks fuller straight away, then settles over about 2 weeks. Skin-quality work builds over about 4 to 12 weeks, and pigment fades slowly over months rather than sessions.

Sessions

Filler is usually a single appointment with a review at 2 to 4 weeks. Microneedling and platelet courses run about 3 to 6 sessions, 4 weeks apart. Numbing cream is used for needle work.

Downtime

Expect swelling and possible bruising for a few days after filler or platelet injections, and the under-eye area bruises more readily than most. Microneedling leaves redness like a mild sunburn for 1 to 3 days.

Aftercare

Keep the area clean, skip makeup for the first day after needle work, avoid strenuous exercise and alcohol for a day or two, and wear sunscreen every morning, because sun undoes pigment progress faster than anything.

The honest part

Fluid puffiness and eye bags from herniated fat will not change with any of these treatments. If that is your main concern, we will say so and point you to what does help, including a surgical opinion where it is warranted. Pigment dark circles are managed over time, not erased in a session. Results vary from person to person, and your physician will tell you what is realistic for your skin.

Every tired-looking eye has a reason. Find the reason, and the right treatment becomes obvious.

The BioPlus approach to under-eye concerns

Questions patients ask

Under-eye concern FAQs

How much does under-eye filler cost in Vancouver?

Cost depends on how much filler is needed and on the plan, so honest pricing starts after assessment rather than before it. Under-eye filler is usually quoted per syringe, and a tear trough often needs only half to one syringe. Your consultation at BioPlus is complimentary, and you leave it with a written plan and exact pricing, so you decide at home. Treatment is a private esthetic service and is not covered by MSP.

Does filler fix dark circles?

Only some kinds. Filler treats dark circles that are caused by a hollow, the tear-trough groove that casts a shadow, by lifting that groove so it stops shadowing. It does nothing for dark circles caused by pigment in the skin or by fine vessels showing through thin skin, and it cannot flatten eye bags. Because pigment and thin-skin darkness are so common, especially on deeper skin tones, filler is the right answer for some people and the wrong answer for others. The assessment is what tells the two apart.

Why are my dark circles not going away?

Usually because they are being treated for the wrong cause. Dark circles have several causes, pigment, visible vessels, a structural hollow, eye bags and fluid, and a treatment aimed at one does nothing for the others. Pigment circles do not respond to filler. Hollows do not respond to creams. Eye bags respond only to surgery. If your dark circles have not shifted, the most useful next step is an assessment that names which cause or causes you actually have, so the treatment finally matches the problem.

What causes dark circles under the eyes?

Dark circles come from one or more of five causes: extra pigment in the skin, fine blood vessels showing through thin skin, a structural hollow in the tear trough that casts a shadow, bulging fat that forms eye bags, and fluid puffiness. Pigment circles are brown and stay dark when the skin is stretched. Vascular circles are bluish and fade under gentle pressure. A tear-trough shadow changes with the lighting. Eye bags bulge. Fluid comes and goes. Most people have a combination, which is why a proper look matters.

Why are my dark circles worse than other people's?

Often because of your skin tone and your genes, not your habits. Pigment dark circles are more common and more visible in medium and deeper skin tones, and in people of South Asian, Middle Eastern and East Asian background, and they frequently run in families. For many people they are constitutional, meaning they are part of how the skin is made rather than a sign of tiredness or ill health. That also changes the treatment, because pigment on deeper skin is treated conservatively to avoid provoking more pigment.

Is tear-trough filler safe?

Tear-trough filler is safe when it is placed by an experienced medical injector, and it is one of the more demanding filler treatments, which is exactly why the injector matters. The under-eye area carries blood vessels connected to the eye's own circulation, so the treatment calls for small volumes, careful technique and often a blunt cannula rather than a needle. Placed too shallow, filler can leave a faint bluish tint, which can be dissolved. Hyaluronic acid filler can also be dissolved entirely if needed, which is part of its safety. At BioPlus this treatment is placed by a physician-led team for these reasons.

Do microneedling and PRP help dark circles?

They can help the thin-skin and skin-quality kind of dark circle, with modest and gradual results. Microneedling thickens and firms thin under-eye skin so the vessels beneath it show less, and platelet therapy, made from your own blood, can improve texture, fine crinkling and some pigment over a course of sessions. The evidence for both under the eye is real but modest, and mostly from small studies, so we set expectations honestly. Neither fills a hollow, and neither is a strong treatment for deep pigment on its own. They are often combined for a better result.

Do I need surgery for eye bags?

If your eye bags are caused by fat pushing forward, surgery is often the only thing that truly flattens them. Creams, lasers and filler cannot remove or reposition herniated fat, and filler around a bag can sometimes make it look worse. The surgery, a lower-lid blepharoplasty, is done by an oculoplastic or plastic surgeon, not at BioPlus. What we can do is examine your lower lids, tell you honestly whether fat is the cause, and point you to the right specialist rather than sell you something that will not work.

Is laser safe for dark circles on darker or Asian skin?

It can be, with the right choice of treatment and careful settings, but under-eye pigment on darker skin is treated cautiously because the risk of causing more pigment is real. The pigment cells in deeper skin tones react strongly to heat and injury, so an aggressive laser can leave post-inflammatory darkening in this thin, delicate area. That is why BioPlus starts with sun protection, pigment-directed skin care and gentle, non-heat options such as microneedling and platelet therapy, and uses energy devices conservatively and only after a test. Thirty years of treating pigment across every skin tone is what makes that judgment reliable.

How do I get rid of puffy eyes?

It depends on whether the puffiness is fluid or fat. Fluid puffiness, the kind that is worse in the morning or after a salty meal or a poor sleep, responds to sleep, sleeping with your head slightly raised, cutting salt and alcohol, and controlling allergies. It is not something a treatment fixes. Puffiness from bulging fat, which is steady rather than coming and going, is an eye bag, and that is a surgical matter. The assessment sorts out which one you have so you do not spend money on the wrong fix.
